receive a price quote for the Chamber’s Electricity Savings Program, please
download the Letter of Authorization. This Letter of Authorization does not permit the Cincinnati USA Regional Chamber to sign or execute any contract
on your behalf. It simply allows the
Chamber to develop a customized savings and pricing proposal for your company.

Once completed, fax this form along with one month’s full statement of
your most recent utility bill for all of your accounts to
513-579-3101. Please be sure to include one month's
full statement for all of your accounts so we can process
your information as quickly as possible.
A Chamber representative will contact you shortly after receiving your
