Application Completion Instructions 

  1. Download and save the Leadership Cincinnati USA application form (MS Word)
  2. Complete application, sign a hard copy of the document and submit in PDF format.
  3. Please include a resume (no more than two pages in length) with your application.

Reference Completion Instructions

  1. Download and save the Leadership Cincinnati USA reference form (MS Word)
  2. Reference should complete the form, sign a hard copy of the document and save in PDF format.
  3. Completed form should be returned to applicant for submission with application.

Application Fee

A nonrefundable application fee of $50.00 is due with your completed application. You may pay via the following methods:

With your application, please submit your digital headshot, in business attire. Photo must be in color, JPEG format, 300 dpi. Submit photos to Dan Hurley at dhurley@cincinnatichamber.com.
