Application Completion Instructions

- Complete application, sign a hard copy of the document and submit in PDF format.
- Please include a resume (no more than two pages in length) with your application.
Reference Completion Instructions
- Download and save the Leadership Cincinnati USA reference form (MS Word)
- Reference should complete the form, sign a hard copy of the document and save in PDF format.
- Completed form should be returned to applicant for submission with application.
Application Fee
A nonrefundable application fee of $50.00 is due with your completed application. You may pay via the following methods:

With your application, please submit your digital headshot, in business attire. Photo must be in color, JPEG format, 300 dpi. Submit photos to Dan Hurley at dhurley@cincinnatichamber.com.
